Hearts&Hands Personal Planning Program
The Hearts&Hands Personal Planning Program begins with a Comprehensive or
Screening Assessment. A Comprehensive Assessment consists of a detailed
review of the physical, emotional, social and environmental issues that affect
your loved one or client. A Screening Assessment focuses on a single
need or concern. Our conclusions are then reported and discussed with
the primary caretaker of the family within five days of the assessment.
Areas of concern will be identified and solutions provided.
Hearts&Hands Personal Pathway Program
The Hearts&Hands Personal Pathway Program offers assistance to your family
member who is returning home from the hospital or skilled nursing home or
when moving to a facility when residing at home is no longer possible.
In both cases, an evaluation report is provided within five days and includes
specific recommendations for each area of concern. In the case of choosing
a facility, alternative sites will be provided that meet the specific needs
of the individual.
When choosing a nursing home or assisted living facility, Hearts&Hands recommends facilities that best meet the needs of the client. Once a facility is chosen, we will help to organize belongings and will make moving arrangements. Most importantly, emotional support is provided during this transition. We will also monitor following the move to ensure your loved one or client has made a smooth transition to their new environment.
When returning home from rehabilitation, daily living are arranged which may include shopping and meal planning, light cleaning and household services. We can also assist the individual maintain medication schedules and obtain prescriptions and/or equipment. We will coordinate services with home health agencies, if needed.
Hearts&Hands Personal Fulfillment Program
The Hearts&Hands Personal Fulfillment Program entails all aspects of a complete
program. Environmental assistance supports the importance of home safety.
Emotional assistance includes companionship, sharing interests and reassurance
to the individual. Social assistance can help your loved one participate
in the activities that have previously brought them pleasure, but which have
been denied due to physical or transportation limitations. Physical
assistance aids in activities of daily living, accompaniment to physician
appointments and compliance with physician's orders and with medication requirements.
Activity reports are provided on a monthly basis. Staff members are
always available to speak with family members or custodians. Of course, urgent
concerns will be communicated without hesitation.
