What is Hearts&Hands Personal Assistance? 
Hearts&Hands is a geriatric care management company that provides one-on-one assistance to the elderly, their families and custodians.

Why should I use your program? 
Providing appropriate care and making quality decisions for your loved one takes training, clear judgment and the wisdom of experience.  We offer a full service approach to care to help assure that everything will be handled in accordance with the family's wishes.  Hearts&Hands will provide you with a plan within five days of an assessment, and will discuss recommendations before any service plan is initiated.

Our highly trained care managers create individualized service plans, focusing on the needs of your loved one.  We use personality tests to ensure our care managers work well with the elderly.  We perform background checks on all our care managers.  We are insured and bonded.

How can you help me with care for my aging relative? 
Hearts&Hands will assess, plan and arrange services necessary for the functional and lifestyle needs of an aging individual.  We can assist you with finding solutions to maintain their independence in their own home.  If living at home is no longer possible, we can help find the assisted living facility or other residence that will be just right for them.

What are your programs? 
Personal Planning Program provides a comprehensive or screening assessment. We complete a detailed review of the environmental, social, physical and emotional needs of the individual. This review will be discussed with you within five days of the assessment. We will identify the areas of concern and our solutions for those concerns.

Pathway Program offers assistance to the elderly family member who is returning home from the hospital or skilled nursing home, or moving to a facility when home independence is no longer possible. We can help you choose the right environment for your loved one.

Fulfillment Program carries out the plan, environmental assistance supports the importance of home safety; emotional assistance can provide companionship, sharing pleasures, and reassurance; social assistance can help your loved one to once again participate in activities that have previously brought them pleasure, but which they have been denied due to physical or transportation limitations; activities of daily living, accompaniment to physician appointments, compliance with medication and physician orders are part of our physical assistance. Keeping you informed is important, therefore we provide you with monthly reports and are available to talk with you at any time.

How will I know services have been provided? 
In addition to the regular monthly detailed reports, Hearts&Hands is available at any time to discuss the service plan with designated family members.  We coordinate and supervise all services provided in the home.  Our management staff routinely visits your loved one to ensure the service plans are working well.  You will know you made the right decision when you see and hear the difference in your family member's well-being during your visits and phone conversations.

How often are services re-evaluated? 
We understand the changing needs of the elderly.  Our care managers evaluate the service plan at every visit.  If new concerns arise, the care manager will communicate a new plan to the family prior to implementation.  If the issue is urgent, a call will be placed to the responsible party for immediate intervention.

Are you associated with any other providers of care? 
Hearts&Hands has no association with other care providers.  Since our inception, we have worked with many companies and resources in the community. We refer only those organizations with the highest of standards. Recommendations are strictly based on the assessed needs of the individual. We routinely visit Skilled Nursing Facilities and Assisted Living Facilities.

What are the advantages to having services arranged? 
Our objective, detailed assessment covers the physical, emotional, social and environmental needs of each individual.  With this assessment, we can identify areas of concern and create service plans that will promote independence and wellness.

Because we are familiar with the community and its resources, we are able to ascertain the best facility to suite your loved one's needs.  Hearts&Hands is proud of its ability to provide and coordinate its services effectively.

What happens if I wish to terminate services? 
Service may be terminated at any point in the service plan with fifteen days notice.  As a final step in the service plan, Hearts&Hands will assist in establishing strategies for maintaining the client’s functional independence after the withdrawal of the Hearts&Hands' involvement.
